Important Notices Regarding Insurance.
Please review these important insurance changes. Contact our office with any questions you may have.
- Until December 31, 2014, we will continue to file insurance claims on behalf of patients with the following insurance:
- Medicare
- Aetna
- BlueCross BlueShield
- Gulf Coast – WEBTPA
- United HealthCare patients: As of December 11, 2014 we will no longer file insurance paperwork on your behalf. Patients with United Healthcare will need to file their reimbursement request directly to United Healthcare. We will collect payment from you for medical services rendered at the time of your appointment. We will give you all the paperwork you need from our office to submit your reimbursement request directly to your insurance company.
- If you have a HMO version of any of the following: Medicare, Aetna, BlueCross BlueShield, Gulf Coast – WEBTPA, or United HealthCare, please contact our office at (941) 342-9750 before your next office visit.
- Starting January 1, 2015 we will only file insurance claims for patients insured with standard Medicare Plan Part B.
For patients with insurance other than Medicare Plan Part B, you will now file your insurance claim paperwork directly to your insurance company. We will collect payment from you for medical services rendered at the time of your appointment. We will give you all the paperwork you need from our office to submit your reimbursement request directly to your insurance company.
You will need to call the customer service number located on the back of your insurance card to ask how to obtain additional forms that may be required by your insurance company so that you can submit your reimbursement request.
